3. Room plan (column 10)

4. Arrival and departure dates (columns 12 & 14)

5. Arrival and departure times (columns 13 & 15)

6. Special requests (columns 61 & 17). E.g: VIP guest, extra bed/ crib, children, allergies, early arrival/ late departure, etc.
